The FRCPath Part 1 in Clinical Biochemistry is a rite of passage for trainees on the chemical pathology training pathway in the UK, medical and clinical scientist alike. Passing it is a prerequisite for progressing towards CCT and Fellowship of the Royal College of Pathologists, and for most trainees it represents the first major academic hurdle of their specialist career. It is also, frankly, a difficult exam - broader in scope and more application-heavy than many candidates anticipate.
This guide draws on the experiences of registrars and trainee clinical scientists currently in training across the UK, as well as our own observations from working with candidates at Smart-Le. Our aim is to give you an honest account of what the exam involves, what preparation actually works, and what tends to go wrong.
In many ways starting this journey was very similar to when we started the histopathology journey. Put simply: there were not any reliable resources and we’re trying to fill the gap.
The single most consistent piece of advice from candidates who pass on their first attempt is to start doing questions early - far earlier than feels comfortable. Around three-quarters of the paper tests application of knowledge rather than pure recall, and the only way to build that skill is through practice. Do not wait until you feel ready. Use the questions to drive your learning, not to test it at the end. This is very similar to the approach we advise for histopathology.
Part 1 leans heavily on analytical techniques and instrumentation, method principles, and their sources of error, this is where candidates from a purely clinical background often lose the most ground.
This is what Smart-Le is all about, that is, active-learning. Make it a key part of your approach including through Anki and other strategies.
Metabolic medicine and endocrinology are the areas most commonly underweighted by candidates, particularly the biochemical basis of less common inherited metabolic disorders, which the exam tests in more depth than what most of our days focus on.

The FRCPath Part 1 in Clinical Biochemistry consists of a single three-hour paper of 125 Single Best Answer questions, set across broad topic areas aligned with the curriculum: laboratory management competencies, analytical techniques and instrumentation, analytical methodology, the chemical pathology of disease (biochemical basis), the chemical pathology of disease (diagnosis and principles of management), and the chemical pathology basis of metabolic medicine. The pass mark is set using the modified Angoff method, and roughly a quarter of questions are knowledge-based, with the remainder testing application. We have broken down our question-bank into these discrete topic areas to facilitate targeted revision.
Before committing to a study schedule, spend a week doing a broad assessment of your baseline knowledge across the major topic areas. Identify your weak areas clearly, whether it is renal biochemistry, endocrinology, metabolic medicine, analytical science, whatever they are, and build your plan around addressing those first rather than consolidating what you already know.
Marshall and Bangert's Clinical Biochemistry: Metabolic and Clinical Aspects is a reference some trainees find useful for the depth and breadth the exam requires. Candidates also commonly use dedicated FRCPath Part 1 question-based revision aids alongside it for exam-style practice (FRCPath Clinical Biochemistry Part 1: A question-based revision aid), and the RCPath's own sample paper questions are essential for calibrating to the exact style and difficulty of the real thing.
Most candidates who pass on their first attempt have studied consistently for at least three months. A useful structure is to dedicate the first month to analytical science and laboratory management - the technical foundations that underpin every clinical question. The second month works through the disease-based topics systematically: renal, liver, bone and calcium, endocrine, lipid, and metabolic medicine. The third month is for consolidation: revisiting weak areas, reviewing every question you got wrong, and doing timed mock papers under exam conditions.
The FRCPath Part 1 in Clinical Biochemistry is passable with the right preparation, but the right preparation requires more than good intentions and a reading list. It requires consistent effort over several months, honest self-assessment, and a willingness to do the unglamorous work of practising questions repeatedly until the reasoning becomes automatic.

How many hours a day should I be studying? Most candidates who pass are putting in three to five hours of focused study daily in the months before the exam. The total volume matters less than the consistency - regular daily study over three months is more effective than intensive cramming in the final weeks.
